Evaluation of Nursing Courses

At the end of each quarter, nursing students are given the opportunity to provide feedback for the instructor.  The evaluations are not available to be viewed by the course instructor until all final grades are submitted to the registrar.  There are two portions to each evaluation. 

Part 1: Electronic Portion:  This part consists of  questions to which the student chooses an appropriate response, and it is entirely online.  Only group data are reported to the instructor as a summary.  At the completion of the evaluation process, you will be given the opportunity to print verification that you completed the process.  You must provide this form to your instructor at the Final Exam to demonstrate that you completed  the evaluation.  (Think of it as your "ticket" for admission to the Final Exam.)  Please click on both links below (theory and clinical) and complete the evaluation process for theory and clinical.  The password is the same one used to access the nursing website:

Part 2: Narrative Portion: This part consists of questions that require a narrative response.  To complete this portion of the evaluation, please click on the following link, download the form and print or type your responses.  The evaluation will be placed in a manila envelope at the final exam.  The envelope will then be sealed and student-delivered to the Director of Nursing.  It will be collected by the course instructor after final grades are submitted to the registrar.   To obtain this form please click here: Student evaluation form
