Information for Students Precepting at the VACongratulations! You will be precepting at a very exciting medical center. Before you get started, though, there are several requirements you must complete. Your attention to detail is appreciated.
First, please download the following attachment (the VA "0711" form), complete Section I (applicant), and sign/date it after you print it. This form will be one of several forms to assemble and give to me:

There will be an appointment for students precepting at the VA to be fingerprinted, obtain photo ID, etc. at TBA (probably week 2 of the quarter). Exact location at the VA is TBA. VA Orientation Agenda

You must bring identification to the VA fingerprinting day. Please click and view guidelines for appropriate identification: VA Identification Requirements
To summarize, you are required to bring to clinical orientation on Wednesday of Week 1 of the Quarter:
- Two certificates of completion from the cybersecurity course (one for privacy, one for cybersecurity)
- 0711 form, signed and dated
- I9 Form (complete Section 1)
On (date and time TBA), bring to the VA:
Two forms of identification (e.g., driver's license, social security card), or more documentation if not a U.S. citizen (contact instructor for more information)
More Useful VA Information: Please be aware of the following VA policies: VA Impaired Nursing Service Employees and Student Nurse Illness or Injury I especially recommend you print and keep the SN Illness/injury policy in your preceptorship packet. Unfortunately at this time, fngerstick blood glucose monitoring by students is not permitted at the VA, due to their competency training and extensive tracking system.
