Tutee ApplicationAll students must submit a Tutee Application to receive tutoring.
Online students or students who cannot come to campus may download, complete, then e-mail the application documents from the links below.
We will confirm that you are enrolled in the De Anza class for which you are requesting tutoring before processing your application.
Either print out and complete the application form, then submit it to S-43 (for Math/Science) or L-47 (all other subjects.), or send the application as an attachment to dowlingpatrick@fhda.edu .
